Question Increased alpha-fetoprotein - which anomaly?

Anne is 16 weeks pregnant and undergoes maternal serum screening to test her maternal serum alpha-fetoprotein (MSAFP) levels. Her MSAFP level is 4.5 multiples of the appropriate gestational median (MoM). Assuming Anne is carrying a single fetus and her pregnancy is correctly dated for gestational age, for what disorder should Anne be offered additional testing?

a) Gastroschisis
b) Pulmonary hypoplasia
c) Renal agenesis
d) Trisomy 13
e) Trisomy 21

a) Gastroschisis

MSAFP above normal is seen in following conditions:
- multiple gestation, placental abruption
- fetal abnormalities - open neural tube defects like anencephaly, spina bifida and abdominal wall defects.
- error in gestational date
- methylene tetra hydro folate reductase genetic variant mothers hav high levels.
- rarely endodermal sinus tumour or germ cell tumours in mother or fetus.

Decreased AFP - downs (trisomy 21) and edward (trisomy18)
