Post anticancer drug with contraindication

q) which of the following anticancer agent is contraindicated in children suspecting of ALL/AML?

a) cytarabine
b) l-asparaginase
c) methotrexate
d) cladribine
e) procarbazine
f) 5-FU

yes, anders is correct ...E is the right answer

procarbazine, an alkylating agent is contraindicated in all leukemias...since it has leukemogenic potential...it alkylates DNA..it is used in hodgkin's lymphoma....s/e--pulmonary toxicity,hemolysis, leukemia, neurotoxicity

cytarabine( pyrimidine analog-inhibit DNA polymerase),
asparaginase( converts l-asparagine to aspartate and ammonia depriving cell with leukemia),
methotrexate( folic acid analog-inhibit DHFR) ,
5-FU
( pyrimidine analog- inhibit thymidylate synthase) all can be used as treatment for ALL/AML....
cladribine--purine analog resistant to degradation of adenosine deaminase....used for hairy cell leukemia....nowadays it is also used for all B cell leukemia's

so, procarbazine is the most appropriate answer------all alkylating agents have potentila to cause leukemias....most common with procarbazine and nitrosureas...
