Question Throid gland

20yr primigravid woman at 10 wks gestation involved in motr vehicle accident brought to ER,she has had no prenatal care.temp:100.8 F. Phsicl examination shows restlesnes,tachycardia and tremor. 1 hour later she is delirious,reports abdominal pain and vomiting.which of folowing is most likely diagnosis??


a) subacute thyroiditis
b) folicular CA
C) Graves disease
d) hashimoto thyroiditis
e) papilary CA

C) Graves disease
answer is c)Graves disease: thyritoxicosis crisis ocurs most comonly in patients who are untreated or inadequately treated.onset is abrupt,and may be preciptated by stresor eg:infection,trauma,childbirth,radio-iodine treatment.
thyritoxicosis crisis: present with fever,abdominal pain,vomiting,delirium,tachycardia,sweating,it is comonly seen in patients with graves disease

folicular and papilary carcinoma: dont produce hyperthyroidism,they are usualy non secretory conditions


subacute and hashimoto's thyroiditis can cause transient hyperthyroidism but less severe than thyritoxicosis crisis
