Its abdominal aneurysm
cause-
Atherosclerosis (most common cause)- it cause destruction and thinning of media.
other cause is familial (but i don't think its the cause in this case)

it's A) intimal streak........ for atherosclerosis( abd. aneurysm)....

intimal tear for aortic dissection (dissecting aneurysms)
monckeberg sclerosis involves media,but this one looks like atherosclerosis which involves intima.....

they are asking how it started


medial degeneration commonly is seen in Marfan's leading to thoracic aortic dissection/aneurysm

Default Answer is intimal streak.

Atherosclerosis is the most common cause leading to abdominal aneurysm. Atherosclerosis starts with intimal streak, which is formed by foam cells. Then more macrophages and lymphocytes move to the damaged site aiming to eliminate the oxidized LDL. Vascular smooth muscle cell proliferates, together with fatty materials and foam cells to form fibrous plaque. Fat deposited in blood vessel walls and proliferation of smooth muscle cells make blood vessel lumen narrows, reducing blood supply.
