Generally speaking you should not Finalize a Date until You have done one round Of the your main books.
you have a lot of things still to do which usually takes little more time then you think.
You havent mentioned the nbme forms in your plan . Atleast one of them online is important to assess the readiness for Exam. Plan it too.

your plan sounds good but, things always dont go as planned. You may need some extra time for some subjects or for qbanks etc.. Some people go thru kapan notes 2nd time, from your plan i think you are planning to do only once. It depends on your previous knowledge and confidence whether to go for a 2nd read or not.. some people do a revision of u world aslo ..
Planning things in advance is not always simple, As of now plan looks good . Take an NBME after FA. You will know if you can take it dec or not, you can change your plan accordingly.. But nbme after uworld would be a better predictor as uworld is a very good source of knowledge.. I personally feel may be you need a little more time, but depends on how much knowledge you have from ur med school and how well you are preparing these 3 months... Dont worry much about the plan right now, complete the syllabus and you yourself will know your weak areas n how much time you need to get the desired score...
all the best
