Drug Oral Hypoglycemia Agents in Gestational Diabetes Melitus

Oral hypoglycemics are contraindicated in GDM due to which of the following

A. GDM is resistant to oral hypoglycemics
B. They are teratogenic
C. They cause rebound hyperglycemia
D. They cause fetal hyperinsulinemia & hypoglycemia
E. They have high 1st pass effect

D. They cause fetal hyperinsulemia and hypoglycemia.

Now I am confused...I am sure that treatment with insulin during pregnancy will cause hyperinsulemia in fetus and then hypoglysemia for a few days after birth but is it true for oral hypoglycemics as well? Not sure.

i'll go with D too

coz...oral hypoglycemics can cross the placenta and cause hypoglycemia in fetus..leading to increased risk of macrosomia and sacral agenesis(caudal regression)

whereas insulin cannot cross placenta....but may cause hypoglycemia immediately after birth due to beta cell production of insulin in fetus
