Arrow Belindalimm Daily Questions: Pathology #13

A 34-year-old Caucasian male presents to your office with periodic abdominal pain. Upper gastrointestinal endoscopy shows an ulcer in the distal duodenum. His basal gastric acid secretion is high with the ratio of basal to maximal acid output of 0.7. This patient's condition is most likely caused by which of the following?

A. Stress-related mucosal erosions
B. Chronic gastritis with antral sparing
C. Chronic antral-predominant gastritis
D. Peptic Ulcer disease

neoplasia does not occur in the duodenum(option G)

As there is increased acid production. So will go with G. Non gastric neoplasm

Because lab result shows high maximal acid output and gastrointestinal endoscopy reveals ulcer in gastric tube, we assume the ulcer results from excessive gastric acid erosion. But the ulcer locates in distal duodenum, we don't think it is peptic ulcer disease which usually occurs at lesser curvature of stomach and proximal part of duodenum. Chronic gastritis with antral sparing means Type A chronic gastritis which involves fundus and body of stomach. Chronic antral-predominant gastritis is caused by H.pylori infection, usually won't induce acid excessive secretion.

Zollinger-Ellison syndrome is a condition in which there is increased production of the hormone gastrin. Usually, a small tumor (gastrinoma) in the pancreas or small intestine produces the high levels of gastrin in the blood
