Default about anemia

in pass program : the most common cause of "iron deficiency anemia" is poor intake ( Dr.francis insists on that it's not due to GI bleeding) while Dr.Goljan mentioned that the most common cause of "iron deficiency anemia" is GI bleeding... so which is right??

also in Pass program : in alpha thalassemia : Hb H and Hb Barts are elevated when there is four gene deletion while in Goljan lectures : Hb H is elevated when there is 3 gene deletion , and Hb Barts is elevated when there is four gene deletion .... ?!!??

Dr Najeed is deffinitely getting something mixed up.
MCC of iron def anemia is blood loss esp from GI bleeding

3alpha gene deletion--->beta chain forms a tetramer(HbH)& severe anemia
4gene deletion-->gamma chain forms tetramer(Hb barts) & still birth
