Default Guessing answers on the exam + a few words

I'm taking my exam end of October and anxiety levels have been up and down. I dont think there's any possible way of retaining everything 100% because with time and further reading things tend to fade. The big key to being successful in this exam is CONFIDENCE. If you aren't confident with yourself, with your prep, with the top answers you choose, then things do go downhill. That was and still is somewhat is my problem. I think that the second key on the exam is understand the concept and correlations. If you understand the overall idea behind it, then it is much easier to apply. But there are always certain things that need to be engraved in your brain.

So I want other peoples suggestions/and or tips on guessing questions on either the actual exam or on self-assessments (NBME, etc).

Does the actual exam consist of - 'either you know the answer or you dont' type questions ?

Are some questions easily guessable if you forget some concepts going into exam day (for example; certain key words like - Dicarboxylic acidemia)

How many of you have guessed many questions and have come out with really good scores?

I'm assuming that the really hard questions most likely would require guessing. How I go about guessing when I dont know the answer is by understanding what each answer means and then choosing the top 2 and pick the one that flashes out more in my mind.
