Question Cause of hypocalcemia tetany

A middle age Caucasian female presents with carpopedal spasm with low serum free calcium and low serum total calcium. what is the most probable diagnosis.
a) metabolic alkalosis
b) metabolic acidosis
c) hypoparathyroididm
c) malignancy

c) hypoparathyroidism lowers total Ca as well as Free Ca.

Yes in metabolic alkalosis
There is more negative charge in albumin so more free Ca2+ attaches to albumin. That decreases free Ca but Total Ca remain same.

Whereas in this case there is decrease in total and free Ca so its most probably due to
HYPOPARATHYROIDISM.
