Default renal disease

A 52 yr man is found to have hypertension during routine examination. Systolic bruit is heard over L reanl artery. Angiography show discrete 95% obstructive lesion with L RA, Rt RA is normal. R kidney is 11.5 cm and left kidney 8cm. which of the following descibed most likely response to treatment?
A Decreased plasma renin activity and decreased angiotensin1 concentration following adminsitration of loop diuretic
B Decreased plasma renin activity following administration of ACEI
C Increased plasma renin activity and decreased angiotensin 1 concentration following adminstration of loop diuretic
D Increased plasma renin activity following administration of ACEI

i'm thinking strange i guess...........furosemide stimulate PGE release--vasodilation (so further decreases renin angiotensin system feedback...)

either A or D(may be)

You should be right.I didn't know Furosemide stimulates PGE release .moreover,this guy should not be given ACEI in Renal artery stenosis

ya, that's the reason i answered A....and also abt. furosemide action--it's high yield buddy..given in F.A too....(but i'm thinking abt there is normal kidney too for compensation....)

D Increased plasma renin activity following administration of ACEI

ACE inhibitor is contraindicated in bilateral renal artery stenosis or unilateral stenosis in solely functioning kidney so it's not the case here

I think ACEI will inhibit AT-2 but renin level don't decrease.
D Increased plasma renin activity following administration of ACEI
