Question granuloma formation..?

Which of the following cytokines are helpful in formation and maintenance of granuloma respectively..?

a) TNF -alpha ; IFN-gamma
b) IFN gamma; TNF -alpha
c) IFN-gamma; IFN-gamma
d) TNF-alpha; TNF- alpha
e) none


i know this q. looks basic..but i really wanna know the true answer of it..i found varied answers from different Q banks....so i raised it into a question....please do give your valuable explanations...........thanks

TNF-alpha is required for the formation of granulomas by macrophages...............For the maintenance of granulomas, IFN-gamma is required...............that's why we have a test k/a Quantiferon test(IFN-Gamma) in tuberculosis.............thanx.

@DrSk----ya, i'm thinking in the same way as yours.......according to uworld

@subbi----this is the answer i came across from other qbank (i think kaplan qbank ,but not sure)..

but goljan and pathoma says...it's IFN-gamma that both forms and maintenance the granuloma..that's the reason i raised this question....pls give ur opinions........thanks

granuloma: acellular center surrounded by activated Tcells and macrophages -> therefore I would go with B

Default b) IFN gamma; TNF -alpha

TH1 secrete INF-gamma, which activate macrophages. Then macrophages produce TNF alpha which induce and maintain granuloma.
This explains if someone in on anti-TNF alpha med (like-infliximab) will instabilize granuloma and lead to disseminated disease.

Dear venky, In my opinion u should go with TNF-alpha for formation and IFN-gamma for Maintenance of granuloma.................quantiferon test helps to clear the concept of maintenance of granuloma by IFN-gamma and for the role of TNF-alpha in granuloma formation in mouse models, I have a link for you please cite...............thanx............. http://www.educus.com/Journals/R15983898-0.html ...........

TNF-a is what forms it.
IFN-y is what maintains it.

After reviewing my notes, reading two explanations from UW, and look up in FA 2012, pg 248, I have to my conclusion that:

Th1 cells secerte gamma interferon which in turn ACTIVATES macrophages. TNF alpha from macrophages - induces and MAINTAINS granuloma formation.

Therefore, with that be said, the answer is B.

Also, in PreTest Pathology 12th ed, pg 71 question 78: Asked similar question and the answer indeed was gamma IFN.

Question was: pathomechanism involved in the formation of these non caseating granulomas involves the action of macrophages to form epithelial cells by the action of which substance? The answer was gamma IFN.
