Virus Viral RNA in CSF!

A 12 year old boy immigrated from Pakistan complaints of difficulty walking. He gives history of fever headache and feeling weak that lasted for 3 days. He recalls finding bright light irritating and experiencing pain on eye movements. O/E
weakness of right leg extension and right patellar reflex 1+. LP and PCR yield viral RNA FROM CSF. THE CAREFUL HISTORY SHOULD BE HISTORY REGARDING
a) history of travel
b) history of dog bite
c) history of tick bite
d) history of blood transfusion
e) history of drinking contaminated water

My answer is E). I think this boy has poliomyelitis.

How do we explain the painful eye movements and Photophobia in Polio?They point more towards Rabies.

@lara-ophthalmoplegia and photophobia is non specific finding due to involvement of meninges and brainstem
