Default Behavioral practice question

You are seeing a 15 year old male in the clinic. He has a history of seizures and has been prescribed phenytoin for management. You check a blood level which comes back low, and you suspect that he has been noncompliant. After informing the patient that his phenytoin level is low, which of the following questions is most appropriate?


A) “How often do you miss taking a dose?”
B) “Taking phenytoin every day can be difficult - what's it like for you?”
C) “In my experience, a low level is usually an indication that patients have trouble taking their medicine as prescribed – do you have this problem?”
D) “If, for some reason, you're having trouble taking your medicine, we can have one of your parents manage it for you to make sure that you take it every day”
E) “You seem to have missed a few doses – do you understand the importance of taking it?”
F) “Phenytoin can have a lot of side effects, but if you don't take it you can have another seizure, which would be much worse”

reasonable, respectful and facilitates discussion as oppose to C

more of close ended question. obviously the pt would say NO! everybody lies.

The answer is B...

It's the most open ended way to ask the patient, rather than making the assumption that he is not taking his medication.
