Default nbme 5 concept

old lady..brought to doc cuz shes been losing weight....xray shows large central mass...woman is capable of making a decision but family says the news will hasten the death...what on earth do you do...

tell family its uncomfortable continuing as her physician blah blah

tell patient about xray findings, refer her to oncologist blah blah

tell the patient her results are available for discussion

tell the patient she should be referred to oncologist for second opinion

tell patient the xray abnormal blah blah

Yes.

Duty to inform the patient first.

If you're still unclear about ethics question...Kaplan USMLE Medical Ethics: the 100 cases you are most likely to see on the exam by Dr. Conrad Fischer is a good read! Highly recommended from this forum and everyone else that needed clarification on ethics portion of the exam.

That answer is correct. Read it in Conrad Fischer's ethics book yesterday... unless a patient is really suicidal, you cannot hold back information from the patient about his/her diagnosis. Goes against patient autonomy. The mantra is, patient autonomy is always most important.

If the patient were to herself say she didn't want to hear the diagnosis, the doctor would then ASK HER whom to share it with or whom she'd like to appoint to take decisions for her. It always starts with the patient herself.
