Hi Vinaya....
Actually i did Kaplan qbank during my revision from kaplan material in a subject wise mode.Its really good to revise everything.I personally felt 1 month subscription of kaplan was nice.

I heard that in Goljan audio..Typical Sjogren syndrome have anti nuclear antibody pattern of SS-A/Ro and SS-B/La.
SS stands for Sjogren Syndrome.

hey guys . i just did nbme 11 and scored 350 (186) ... my exam is on aug 28 ... i've been studyin fa and uworld .. really depressed and freaked out ..

do u think i'll be ready by aug 28? ...

What all have you done till now... Imean kaplan etc etc any qbanks that uve finished and whats ur plan for the coming 50 days .... Thats what ur performance depends on

i did kaplan long back initially ..but have been doin first aid and uworld for a few months now .. i had taken an nbme (form 6 ) on may 1st and got 340 . had gotten depressed and took a break in may and started studyin again in june ...

have finished u world twice (first time i was gettin around 50% . second time around was gettin in the 70's )

my tentative plan is to read fa and do 2 or so blocks of u world everyday for the next 60 days ..

not sure exactly how to go about it . if u do know or have ny advice it'll be really helpful

Im no expert but this is what i would do after uworld and adding notes onto FA i would revise FA and do an NBME ... That would tell me whether ive done enuf for step 1 ... I would not take an exam if i dont know whr I stand

interesting...this isnt given in FA nor Kaplan i think. I need to check Pathoma though.

hi guys... qs time...

a 6 yr old caucasian male has met with a motor vehicle accident and needs blood transfusion. he has had repeated respiratory infections in the past and diagnosed with igA deficiency. who can donate blood to him...

1) a normal healthy individual
2) a person with igA def
3) a person who has received multiple blood transfusions
4)a person who has not received any vaccination
5) a healthy donor with HLA match

yes that is the right answer.... people with IgA def have anti IgA antibodies and lead to anaphylaxis on given a normal person's blood....

Whats the mechanism of action...

a.Anti angiogenic effect of IL-2
b.IL-2 induced apoptosis of tumor cells
c.Enhanced activity of NK cells
d.Direct Cytotoxic effect of IL-2
e.Increased expression of MHC-1 on tumor cells

Il 2 is responsible for activation of cd4 t lymphocytes which then induce the appropriate responce which can be mpg activation or b cell activation or in this case nk cell activation (although it is a part of innate immunity it does get activated here)

This is a tricky one, no doubt. I dont think its e, because IL-2 increases expression of CD8+T cells which associate with MHC-1. The tumor cells are renal cells, not the T cells. Its not b, IL-2 doesnt induce apoptosis, thats CD8 again. Its probably C, because i looked up IL-2 on wikipedia and it says that it plays a role in differentiation and proliferation of NK cells, like IL-15. But to be honest im not sure. EDIT: Looks like the answer is C.

My exam is in 4 weeks ... Here is what im doing... Uworld with revision on important concepts and new concepts... Plan to do NBMEs ... Ive already done NBME 13 and then im going to revise FA till the exam (mostly last10 days before the exam) ..
Since youve taken nbme i recommend u revise the areas you were weak in before doing uworld... And then use uworld to consolidate...

48 yr old ,3 day history of fever,2 week colicky lower abd pain, p/e left lower quadrant pain, feverish, normocytic normochromic anemia, leukocytosis with left shift. treated with iv antibiotics, ready to go home after 3 days, what is the advise for follow up?}
a.Avoid Alcohol
b.Use stress reducing activity like yoga
c.Initiate high fibre diet
d.Join suport group with similar condition
e.Walk atleast 2 miles daily

hi..since u have already taken a nbme ...what did u complete u complete before the nbme?...were u done with full u world...im asking because im planning to take a nbme next week and im scared that a low score will dishearten me and i might detoriate post that...

I had done only kln and FA along with the kaplan qbook. Thats all. I started with uworld after the nbme... Plan to finish uworld in a weeks time and then do uwsa and more nbmes

That wasnt one of the options ... Its either related to metabolism or absorption of the two and mostly its metabolism but dont know wht type

Question was something like a patient is given statin and fibrate and develops rhabdomyolysis... The effect of fibrate on statin best explained by
A ) inhibition of cyt p450
B) increased absorption of statin
C) increased glucurinidation

I checked wiki it says statins metabolised by cyt 450 so i guess it must be the inhibition of the same leading to the increased toxicity

altho statins are metabolized by cy450 ....fibrates dont effect cyt 450 as far i know .. out of the options u;ve given so far none of them seem likely . it may have been one of the other 2 u said u dont rememeber ...

in animal experiments insulin conc in pancreatic veins is measured... epinephrine is found to decr insulin secretion and then with addition of drug A it shows paradoxical incr in insulin secretion. which of the following could be drug A--

1) propranalol

Where have you scheduled?

I read Kaplan twice so far. FA once. Goljan once. Did 40% Uworld with around 63% cumulative.

Now doing 3rd read doing the remaning QBank questions and revising FA.

I think it's 3)Phenoxybenzamine-Alpha blocker.That block alpha receptor mediated decrease in insulin release and expose beta receptor effect of epinephrine that increase in insulin release.

alpha2 ---decreases insulin secretion
beta2 --- increases insulin secretion

because ....at high doses of epinephrine ....Alpha receptor activity is more stimulated then the Beta receptors ....so phenoxybenzamine inhibits alpha activity ...hence more beta stimulation resulting in increase in insulin secretion

i've scheduled in dubai ...... i've just been doin fa and uworld ...worried tht might not be enuf .. i did kaplan once 9 or months or so ..havent touched it since then ...

FA done cover to cover takes care of 90% of the concepts... Uworld is the best qbank to consolidate those concepts and give u an idea of how each concept can be tested... Your doing great.... Good luck for the exam

Since the main pathophysiology is pumping against increased resistance it should be concentric hypertrophy with little changes in overall size... On the other hand a volume overload will cause increased overall size with thickness of ventricular wall

seems like all r pretty busy with the final GO....

Does any1 know if there r ECGs in the Exam?? I mean i have done 50% of uworld and i never got a question with an ECG in it...?

Yes im sure there are questions related to ECG on the exam .... Even in uworld ive come across a few very good questions mainly related to cardio pharma and ecg... Also i noticed that some questions that uworld added around may has a lot of pictures and graphs ... Hope thats not due to the new question pool

Hey .. Depends on your prep... If youve done kaplan and FA atleast once aim for 220 +... If youve done uworld aim for 240 +..

...guys...I can't seem to get the hang of things on pharmacology...I consistently and persistently fail the pharm questions in UWorld, what can I do to retain all these drug names, side effects as that's my major problem...it is very frustrating as I do not know what to do...help!!!

Hey guys, long time lurker first time poster. Hoping to take step 1 in August as well. Took NBME 11 on 5/31 and got a 400, took NBME 12 on 7/5 and got a 440 (207). Looking for advice on what to do with one month left. I have already done Uworld, Kaplan, Qbank, and USMLERx, just reading over my heavily annotated FA for now with pathoma which is quite exhausting, I want to do more questions and may try USMLE weapon, but I'm not sure. Look forward to communicating with you all. Please feel free to ask me any questions, my first NBME was a 70/800 I can give whatever advice have to anyone who wants it.

What resources are you using? I find FA to be enough for most pharm questions in Uworld, with a few exceptions. The nemonics and charts make things easier, try giving it some isolated repetition, maybe only doing the pharm sections in FA for 2 days without doing any other subject.

Well after taking NBMEs 6 and 7, there seemed to be a lot of questions from those that I couldn't answer from my FA, however after taking 11 (a newer one) I realized that so many questions on the newer tests are FA based (hopefully step 1 follows this trend). Between 11 and 12 I did 2 reads of FA with pathoma in the corresponding sections. I also have a 1 year prescription to rx and did my incorrect questions in rx. The beautiful thing about rx is not only do they give you practice questions from fa, but the pages from the fa section are in the qbank questions, so this is like doing another read of FA also. Some things I remember from FA just because USMLERx had an annoying question on it.

hey! i am an IMG! when it's the best time to take step 1?i know that AMG take it after their first 2 years, but i feel like there's such a big difference between what i am learning at school and what step 1 asks....sooo, it's best to wait for my last years of meds school (like most IMG do), or should i just do it earlier?(and if so, what year do you find it to be the best moment)...pros and cons?...thanks, anyway...and good luck...

Default First Panick Attack

Hi Guys,

I haven't been too active on this thread since my first few posts, but have been silently following it. I have been studying for the past 9 months for this exam and have been pretty calm and composed throughout. Yesterday, I scheduled my exam for 15th August and today had the first panic attack.

I am done with 40% of Uworld and have maintained a decent average of 80% so far. I did NBME 5 offline and for some weird reason I found it tougher than UWorld.

As far as UWorld is concerned I am happy with my score but what's killing me most is that a lot of questions which I get correct are kind of distant hunches, gut feelings (of having read it SOME WHERE) and by elimination of options, rather than knowing them straight out. Even though only a month is left and I feel I haven't consolidated all the info needed for this exam, things are still not crystal clear and kind of vague. What I figured after doing NBME is that it tests you straight and you eighter know it or you don't. That's the reason why I found it tougher than UWorld.

My plan is to get done with UWorld by the 21st, which will leave me 24 days till the exam. I plan to go through First Aid twice in that period and do NBMEs 7,11,12 offline, UWSA 1 & 2 and NBME 13 & 15 online, in those 24 days.

I'm also feeling a bit uneasy as people stress a lot on annotating from UWORLD and making the most of it as a learning tool. At times I find it tough to decide what to annotate and what not to and so far I haven't heavily annotated a lot onto my FA.

I just hope this feeling of vagueness, uncertainty and uneasiness goes away soon so I can stay focused once again. Also i'd appreciate any recommendations and proposed changes to my study plan. Thankyou guys for starting this thread it has really helped me out a lot!

I heard from one of my friend who has got great score that read as many times as you can.Everyday i spend an hour to read FA and my plan is to read it at least 5-6 times

I just hope this feeling of vagueness, uncertainty and uneasiness goes away soon so I can stay focused once again. Also i'd appreciate any recommendations and proposed changes to my study plan. Thankyou guys for starting this thread it has really helped me out a lot!
I'm so glad someone feels the same as I do. I finished my UW with 78%, and did NBME 15 online about 3 weeks ago. And I was so surprised when I saw my score- 254. Because there were a lot of questions I answered just by my intuition.
So, I panic every morning (because I simply cannot recall all the facts) and it takes me about 2 hours every morning just to concentrate. I keep saying to myself that intuition is a higher form of thinking but it doesn't help.
I suppose panic can be both good and bad, but I would like so very much to achieve that 250+ score!

Same here guys! This is what i tell myself ... No matter how many qbanks or books you go through there will be questions where you will have to make that brain of yours tick and figure out the answer aka intuition.... Nobody who ever takes a test knows all the questions.... Just do what your doing and take this test without panic and you will do well ...


KovachMarina whats your plan for the remaining prep time.... Im doing all nbmes -->will review marked questions from uworld ----> final read of FA

I plan to do UW 2nd time, I've finished about 50% of it, and after that I will do NBME's. I'm also revising FA. Maybe I'll do NBME 13 online, and 7, 11, 12 offline. Maybe all of them offline, I'll see.
Actually my problem is that I'm coming to USA next week, and that will take few days off my preparation. And that makes me more anxious. I'm thinking of postponing my exam (which is on 6th of August) but on the other hand, I'm saying to myself-your NBME was good 3 weeks ago, you should take it!

hey so am doin uworld second time around .. scoring around 70% ... i did an nbme 11 2 weeks ago and scored only 350(186) .. but when i read fa . i feel like i know everything whts in t ,.. (i've read t multiple times) .. but dont know why am scoring so less in nbme ....

Thanks guys,

Good to know that the feelings are mutual! I realized I was going too hard on myself since I hadn't taken a single day off last month. So took some time off today, prayed, spoke to friends & family and watched shows.

My kind of 'negative' post could have been demotivating for a few! Sorry for that, I only intended to share my feelings and hear from others! Just a matter of days now, hopefully we will all ace this test. :sorry:

I must say the August test takers seem to be the most active and the most helpful among all!

What are your weak areas in Nbme? With uworld and FA you should be scoring atleast a 220 on NBME .... Have you done kln properly?

good for you... all of us need a break once in a while... ya i guess the nearer it comes the more we need the help of ppl to keep our cool.... hmmm so whose the first person writing exam here?? aug first week??

on nbme it said i did really in cvs renal repro musculo and histo .. i did kaplan once a year . been studyin on and off since then wid fa and u world ...

IMO not worth giving NBME in 10 days... If you have time i recommend you read kln once and then FA with each chapter...focus on cvs repro msk and histo...trust me this is going to boost your score.... Incase you dont have time read kln for weak areas only and do FA...
Also if your confident with FA but not doing well on NBME try usmlerx qbank... From your post i feel your problem is not knowledge but some difficulty in answering questions .. Practising with a qbank will definately help with that.

Lastly DONT give up DONT loose hope... Work harder and you can ace it

I took my date Aug 29Feeling bit tensed but excited.Finished with 80% of Uworld with the score of 70%.According to you people, when should i go for NBME & which one first?what about you all?

I would suggest,, take the nbme 15 early...its got different type of questions but the score will be pretty accurate...

Ive schedule mine on august 19th...will return for a feedback for the late august test takers...

Nbme 13 - 15
These are the latest and good predictors.. Take then atleast 3 weeks before to know strenghts and weakness and then focus more on weakness to improve scores

plannin on doin 2 blocks a day and readin an fa chapter ... i dont think i have time , my exam is on aug 28 .. ...will try doin usmlerx .. do u knwo whr i can download an offline version ?? ..thnks alot for the advice ..

also how do u guys keep urselves motivated to study everyday . i;m finding it hard i study for 4 5 days and then slack off for 3 days
