Its not extremely common but i wouldn't rule out such x rays appearing on the step 1. Step 1 more commonly uses CT sections/ MRIs/ Gross images and expects you identify a certain something from the question. Step 1 more commonly shows only normal CT and MRIs for you to identify structures you would "expect" to be affected in the case mentioned.

Also i should mention almost all image questions on the step 1 can be answered from the question stem directly, which the image just offering the exam taker a way to confirm his answer if he's unsure, or in some cases the you might have to choose an option based on the image.

Uworld/ goljan slides should be enough. But if you feel uncomfortable still, there's always Google.

If youve taken a few nbme's you will notice that in imaging there are certain things you cant do without ... This is my short list . Hope it helps you
MRI KNEE SAGITTAL AND CORONAL SECTIONS
MRI /CT BRAIN CORONAL AND SAGITTAL VIEW
BRAIN ANGIOGRAMS JUST A ROUGH IDEA IS GOOD
CT ABDOMEN *VERY IMPORTANT* usually asked to identify vessels or structures related to stem but ive seen direct questions
XRAY CHEST- cardiac structures very important... Basic lung pathology - lobar pneumonia ARDS , pneumothorax....
All in all i felt the images very helpful in coming to a conclusion ... If youve done rotations in medicine and surgery you should not have a problem with these....
But mri knee was a pain so i did from the internet
