Default Chronic Inflammation

Hello I understand that a TCR complex recognizes antigen presented on MHC molecules and Activation of T-Cells requires Binding of Antigen/MHC Complex and an additional second signal.

But what exactly is B7 on APC Binds to CD28 on CD4 helper T-Cells providing the 2nd activation signal exactly mean?

I think I'm not understanding this whole B7 thing.
