just did the free 150 questions
came across this question..can anyone plz guide me thru the calculation:
a 70 kg man on a fixed Nacl intake (200mmol/day) is given daily injections of a potent mineralocorticoid for 4 days,he has free access to water and consumes his usual caloric intake.excretion of Nacl is as follows
day1: 30mmol
day2:90
day3:180
day4:200
assuming that 1 litre of 0.9% saline weighs 1 kg and contains 150 mmol of Na cl, how much will this pt weigh in kg at the end of day 4
a.66
b.68
c.70
d.72
e,74

The man consumes a total of 200 x 4 = 800 mmol of Nacl in 4 days. However, he is losing 30+90+180+200 = 500 mmol of Nacl, at the end of 4 days. Therefore, there is a net gain of 300 mmol of Nacl (800-500).

1L weighs 1 kg and contains 150 mmol of Nacl. Therefore, 300mmol should be 2 kg. So, the person should weigh 70+2, i.e. 72 kg at the end of 4 days.

I just took NBME 12 offline, I know its' not a wise thing to do but I only have enough money to take 15 online which I will in ~ 10 days or so.
But, I got 50 incorrect in nbme 12 - i thought, few of those questions were tricky and not one answer could be justified. Does anyone knows what score that would correlate to - ?
I took nbme 13 online on August 20 and scored 214 - I feel like my performance has dropped
I am freaking out now....I also need to build on my stamina

even i found this nbme 12 so hard i donno how ppl have got 240's in it.. its questions freaked me out too

I postponed my exam after your consult, I have been reading first aid and memorizing it from the start, and I bought "Medical ethics for Conard Fischer" to improve my skills on ethics ( which was one of my weaknesses )

I've been reading from FA Q and A. along with FA second run with emphasis on memorizing it,

Also reading my mistakes from Uworld and NBME form 7

sometimes I think I'm going slow !

my exam will be on 18 November

The following question is stupid but I found 2 answers for it

which bacterial structure dose ethanol disrupt :

1- Cell wall

2- Protein

3- DNA

4- membrane

5- RNA


I found answers that says it disrupt lipid and denature protein ??

lipid in cell membrane

any explanation

finally done with the exam..it feels like i watched a dream..little busy, will giv u guys an update soon..for all those who hav to take it, just rember the bottom line, its keeping ur senses intact during the 8 hour period

it "denatures proteins" and "disrupts the cell membrane". This is straight out of Uworld (i just looked up the question talking about this to confirm).

i took nbme 12 offline a week ago n got 32 wrong... i dont know but i am freaking out too bcoz people say nbme 15 is much tougher... my exam is in 15 days time.. nbme 15 in a couple of days.. not knowing wat to do...

Default my exam experience

hi frends,
so here is my experience of usmle step 1 i gave on saturday.
although i cant reveal exact questions as per rules, i will try my best to give u a picture thats helpful for u..
1) regarding uworld: real exam is very similar to uworld, i wud advise everyone to do it thoroughly especially the explanations, dont rush thru them..
2)regarding FA: when i used to read fa i thought y ppl say its so essential for exam, when its so boring to read and cramming material..my experience is that fa is a must read to consolidate watever u have read in kaplan n elsewhere, its not to make concepts..it ensures fast recall on exam day..time is flying on the real exam so the facts shud come to ur mind fast
3)regarding subjects:
micro, pharma, patho was pretty straightforward
anatomyne point i want to stress which nobody told me is that anatomy was heavily tested on my exam..n i didnt do very well on that cz i just read kaplan. so if u are earlier on in ur prep, do read high yield gross anatomy,
i read hy neuro which was sufficient.
behaviour science: as expected was very very bad, no source helped significantly, q were very vague, just made my best guess n moved on
physio: they give a lot of questions in which they describe a hypothetical experiment n then ask u which parameter will increase etc..some r easy, some not.
biochem: not many questions, i read it in a lot of detail, but there werent many q. but every form is different, u might get more questions.
audiovisual questions: 2 ecg, 2 heart sounds n lots of CTs , n pathology gross n microscopy images.
1 ecg n 1 heart sound was difficult, the other was easy, ct questions were ok except 2,3 weird q.pathology was simple.
finally the exam experience: first two blocks were tough n i was nervous also,,after that i synced with the exam n was more comfortable.
the challenge: i felt the real challenge was to manage time, keep ur senses intact for 8 hours, n to steer ur way to the alike looking options,these guys do a very good job to make confusing options, where u even falter at a topic u very well know.
all the best to all of u, just keep ur calm, sleep well before the exam, n pray for me, hopefully i get a good score!!

were you able to get the question without listening to heart sound or looking at the histo pic?? and how was ur biostat part, and what was weird in behavioral?? those doc-patient questions??

even i feel that i am not reading much anatomy for this exam, coz even after we do fa and kaplan, still it looks like not enough.. do u think i should get high yield gross anatomy??

hi abeer,
histo pic questions were simple, q stem was quite leading..but heart sounds n ecg were difficult, q stem didn help much..if u r early on in ur prep then only go for hy anat, its a big book, will confuse u even more if less time is left. behaviour sci questions were those best response types in doc pt relationship, were v vague..biostats some were strtforward calculations, but some were weird graphs thy asked to interpret. u cant do much about that,hope this helps, all the best

After having gone through it - do you think that FA/UW for the last 2 weeks would be wise? Did you do any NBMEs? I actually didn't study anatomy from Kaplan but am usually on the right side of the NBMEs for anatomy and histo sections... I'm not sure if thats okay because I definitely don't think I have time to go through kaplan at this stage.

My exam is in 4 weeks...

Well I meant courageous - I've postponed my exam by like 4 months because I'm so terrified of taking this damn test. The NBMEs just add more anxiety to it... I know lots of people who did one in the beginning of their study period and just didn't take another one because they didn't want it getting in their head.

lol yea! You'll be fine! From what I've read in SDn... when you're at like 265 level..it doesn't overpredict quite as much.

well with 11 days in hand, the only thing u can do is to look at a lot of pictures in hy neuroanatomy or web images..may be u get fewer anat questions than me on the real test..all the best

Review Anatomy from FA...especially in Musculoskeletal section...for upper/lower trunk lessions.

For Neuro, u can go over HY neuro in like a day.
plus from FA know cold: VISION loss
Arteries and its consequences...Like whats gonna happen if we occlude Basilary artery and how its going to present.

IMO neuro is relatively simply on Step 1, comparing to when u read the books
and as aspiringdoc said...just look at online images.
Looks at CT's
KNOW Cranial Nerves cold and able to identify them on the picture...

i did fine in the anatomy sections on U world, it didnt seem too bad. But lately, i have heard the real exam is testing stuff like muscle insertions, something neither FA or U world is covering. I dont even remember Kaplan covering it that well.

yeah thats true, uworld n kaplan didn help much in most of the questions..im not sure if i wud hav read high yield i might hav done better..

sorry abeer but i fuond kaplan enuf for doing those questions, didnt use any other resource..to ease u a bit, i didnt get a single question on that, although i wanted to hav 1..

@aspiringdoc - Congrats on being done with it! 5 days to go for me as well. Just wondering what was your percentage in UWSA2 if yiu remember? Just trying to figure out their scoring as 265 is maximum possible I guess. Thanks!

Abeer, the only thing you need to do for acid-base is to know how to interpret an ABG. Watching the kaplan video might be the best thing to do. It is an important topic so make sure you have it set right for you.

i got 22 questions wrong in uwsa 2, and the score was 265..not sure whether thats the max or not..all the v best to u..u will do well

i watched kaplan videos at the start of my prep but i never liked that guy who taught in it ,.. i mean , fischer or someone else cud've explained it better...

Renal was one of the sections i REALLY liked in kaplan physio... watch the videos if u can

@abeer - Okay, here's what you can do. He explains in too much details. Don't get confused. You may skip the davenport diagram because I don't remember seeing a single question on that in uworld or nbme. Learn to classify situation into the 4 possible diagnosis. Respiratory acidosis / alkalosis and metabolic acidosis / alkalosis. Practice questions once you understand.
Hint - Always look at the ph first and then look at "which derangement is causing the altered ph". For that you must know high CO2 = acidosis and low HCO3 = acidosis etc...

took NBME15 earlier today, ended up getting a 640/254 . I am going to take UWSA 1 on either saturday or sunday and see how i fare. Exam is on the 25th!!

Goodluck Cisterna chyli ....

so after a long pause i am posting again ... as i was ill ...
performed worse in nbme 12 in 230 even i did fair in nbme 11 : 245
now giving a read of Fa ... meanwhile in btw did UWsa 1 .. and was about 252 ... still tensed as i am sort of at the same spot or even i guess going on the downward slope after getting ill ... as uwsa usually over estimates ...
Also i found qs in it quite vague ... Not well written i think ..

may be mmpetrov can through some light on it that is exam qs such vague type ... ???

i am thinking to complete read in couple of days and take nbme 15
wt else guys ? any suggestion .. exam at end if this month

either i hit the goldmine on NBME 15 or i was just in the zone, but i did AWFUL on UWSA 1. The exam was heavy on genetics, molecular bio and behavioral, which are all my weak subjects and it threw me off my game totally, which made me loose focus and mess up the rest of the exam. I decided to postpone my real exam and regroup. I need help on these 3 subjects, are there any good resources for these besides Kaplan?

i think for molecular person cant do much . . . although just looking at robins basic pathology diagrams might hellp as person in DIT said for molecular and cell biology . . . .. but i think its very tiring . . . but upto you if you want to climb that mountain . . .

480/215. I dont know how i did that bad , after doing so well on NBME 15 the other day. The questions were really twisted on UWSA 1, nothing that i have seen before either in the Uworld or USMLERX Q banks, or in the NBMEs. Plus my added troubles with those 3 subjects and it just kind of messed up the whole exam for me. Not sure what to do now....any ideas? I somehow felt like what i had done for the past 4-5 months kinda went all up in smoke on this exam lol...

generally speaking Uwsa over estimates .... But in your case as u got very high in nbme 15 ... yu should consider the avg of your nbme s if you have taken them after doing UW qbank ....
am not expert ... But i think best plan can be to revise FA once and then take another nbme aftrr a week ... or just take another nbme rite away .. if you feel it can be False positive .... Both can be options for you ...

I need help in molecular bio, genetics and behavioral. Is the real exam like uwsa1 or the nbme?? Because if it's like uwsa1 then im definitely in a bad spot

Hello,
My exam is next tuesday, So here is my worry - When should I take nbme 15 - i am trying to do a final read of FA and my notes but that is taking a long time, like 10-15 pages an hour So should i take nbme 15 after i have reviewed everything or take it now so i know where i stand. ALso, my last nbme was in late august - long time right ? nbme 13 scored ~ 214

guys ,I have already extended my eligibility period once nd booked for nxt slot that means nov-jan .My last date to take step1 is 31st jan. my 2nd read will finish by 2nd wk nov nd most likely wd one read of fa ..I just am taking uw notes now wd it(but not reading uw yet except few) that uw note taking will too finish by 2nd wk nov .after that I will get approx. 2 months in hand to read uw, revise ,and more practise.I 'm very nervous as thinking if I will be able to be ready by end jan wd several revisions etc...I forget stuffs too often.guys , do u think it is possible to finish everything by this time or I need to think now to pay 2nd time nd book new slot after jan ?? I work full time for this exam,

Tai35 I'm in the same situation. Last nbme (nbme15) in the end of august, then I started working, only studying part time, more or less...Now I have 14 days free to review...I really hope that my performance hasn't worsened....but I am very scared...
