Default osteomyelitis in sickle cell disease

hello people,

i was going goljan and robbins pathology bone and musculoskeleytal system chapter. In that both of them say that osteomyelitis in a patient with sickle cell disaese is most often due to salmonella paratyphi. Why are they more susceptible to s.paratyphi than normal individuals (s.aureus is the most common cause of osteomyelitis)?

People with SC anemia usually have a hypofunctional spleen, resulting in more susceptibility to infections caused by capsular microorganism . Salmonella being one, poses greater threat for osteomyelitis in such individuals since it can't be cleared efficiently via the spleen.
Remember, such individuals are also more prone to pneumonia and meningitis being caused by Strep Pneumo and N.Meningitidis and thus require prophylactic vaccinations as well.
