According to ECFMG (Educational Comission for Foreign Medical Graduates), the fees of USMLE step 1 as of January 2014 are:

$850 (Fee of the exam) + $150 International Delivery Surcharge = $1000

That's basically the cost of the exam in itself, then you have other expenses such as:

Books (ranges from $50-$500, depending on which and how many books you buy. For Example: Kaplan Lecture Notes + First Aid is around $250, while one review book for each subject + First Aid will end up costing around $400)

Qbanks/Qbooks (Qbanks price depend a lot on the number of months you want to subscribe, but is no cheaper than $100, while Qbooks cost something like $50)

NBME's (Approximately $75 each, not really sure of the price)

Courses such as Pathoma (for Pathology) or the Kaplan courses may be part of your expenses as well, Pathoma costs something like 80 bucks while Kaplan courses' price is astronomically huge.)

The price of the exam is set, but how much money you spend in preparation is up to you and what are your score goals!

If you want a rough cost for steps fees, qbanks, elective costs, airfare for 2 trips, residency application costs, visa etc etc.. I'd say roughly 10 lacs was the figure i was given by Kaplan but you can say it could vary by a couple of lacs.
