Default nbme 11 old lady brought to emergency department after found wandering on street

old lady brought to emergency department after she was wandering on street...she says she wants to go home...whats the best thing to do as a doc?

is it attempt to contact her family?

or take the person home?

I thought taking her to a nursing facility was the right answer but got it wrong

I picked the 1st option. Logically speaking, u need to inform family/get her background information to treat and make sure she gets back home, once treated, safe and sound.

Sending her to nursing facility would be wrong cuz in the USMLE, you are the physician, you need to deal with/cure the patient. So, almost always, sending the patient home, referring the patient to a different place is an incorrect choice.

Hope that helps!

Always remember RULES and apply them:
RULE 18: ALWAYS ELLICIT MORE INFORMATION FROM THE PATIENT
seek information from the patient behind the dz.

Id be cautious of applying strict rules to behavioural questions...ive seen a few kaplan questions which i think specifically aim to catch people out this way...for example a possible HIV patient getting angry at being asked if he has considered getting tested - the wrong answer was to ask him why he felt that way, and there were a few other questions where asking for more info was the wrong answer. Im sure always asking for more info would be the right answer far more often than not, but i wouldnt be surprised if the real exam put some questions out there to really make people think and catch you out like kaplan did. Generally speaking if you just try and put yourself in the position the question asks and think what is the most sensible thing to do, its probably the right answer. Like imagine you are working in the ER and this old lady was brought in, would you really immediately organize to get her to a nursing home she has never been to and who dont know anything about her, without trying every way possible to find out anything about what on earth is going on? Probably not

UsmleUk thanks for pointing that out, i just started qbank and im getting all behavioral Qs right by following the rules as Daugherty suggests..
it may be misleading in some cases so i will take that in account and not be so closeminded with the rules as i was.
