Help U.S vs. Home country ?

I have a dillema and I would like to take your advice.Iam a dermatology resident at my home country.
I love derm but my dream is to train and live in U.S.
I know that derm is impossible in U.S.
Iam thinking of applying to IM residency in U.S.I also like I.M but not much as derm.

Do you think being a dermatology resident will decrease my chances of matching into IM , since they will question my commitment to I.M.

Your advice is highly appreciated.
