Heart Transposition of great vessels

Transposition of great vs. causes right -to-left shunt...and for the fetus to survive must have other cardio protective shunts in the heart too:

ASD steps up O2 in rt ventricle to deliver to tissues via aorta

I dont get how VSD and PDA can help this case?

well, the only thing you need to know is that right ventricle , HERE ONLY, works against the systemic vessels . so ,this makes right ventricle the high pressure system. the rest is easy.

vsd shunts blood from right ventricle to left,so that blood can be sent to lung for oxygenation via pul A. same for PDA that shunts blood to PULMONARY ARTERY for oxygenation.

So the concept here is :"Right ventricles work against the systemic vessels" well that does explain it..although i don't get why the exception in here..golijan doesn't explain it??
Could you elaborate?

oh hey, the exception is because aorta started from the right ventricle instead of lt ventricle.and vice versa for pulmonary artery and left ventricle.

it is a congenital defect. in simple words, imagine originally aorta and pulmonary trunk develops from rt and lt ventricle respectively. then in between there is a "conotruncal" septum. this septum is supposed to take a spiral course and resulting the interchanged position -----aorta from left ventricle, pul A from right ventricle. If it fails-----transposition of great vessels occurs.


i hope i made myself clear. if not, plz go through langman's embryology. i think i read it from robbins....dont remember though.
