Kidney What would you see in renal biopsy?

A 9 year old boy has an upper respiratory virus that improves after day 5. On day 7 his throat becomes very red and sore. He his treated appropriately by his family physician and he improves. On day 16 the boy’s mother notices red tinged urine when the boy voids, and she rushes him to the ER. A microscopic urinalysis reveals urinary sediment with red cell casts. If a renal biopsy were done, what would be seen in the glomeruli?
A - Fusion of podocyte foot processes
B - Linear IgG immunoflouresence along the basement membrane
C - Mesangial IgA depsosits
D - Subepithelial deposits
E - Wire loop lesions

remember that:

Hematuria weeks after an infection = post-infectious glomerulonephritis. Hematuria days after infection = Berger’s Disease, aka IgA nephropathy.

D: Subepithelial deposits --> This is a classic presentation of post-strep GN

Choice A describes Minimal Change Disease
Choice B is Goodpasture's
Choice C is Buerger's or IgA Nephropathy
Choice E describes SLE
