Drug Practice Question: Pharmacology #2

A 67 year old male patient presents to your office with his wife. She is very concerned because she says that her husband has become progressively inactive over the past couple years. He sits on the couch with a blank look on his face and his hands shake when he is not using them. She says that her husband used to be very happy, but now he barely even smiles. You examine the patient and notice that he has a shuffling gait. One of the potential drugs used in the treatment of this patient is also an anti-viral medication that works by which of the following mechanisms?

A. Inhibits DNA-dependent RNA polymerase
B. Blocks attachement/release of virus
C. Inhibits uncoating of the virus
D. Inhibits mRNA transcription
E. Inhibits DNA-dependent DNA polymerase
F. Inhibits Reverse Transcriptase

Option C. Inhibits uncoating of the virus

This patient has Parkinson disease, and the antiviral drug that can be used is Amantadine, Amantadine blocks viral penetration and uncoating, and also can cuase dopamine release from nerve terminals.

The patient presents with signs of Parkinsonís Disease. The treatment that the question refers to is amantadine, which used to be used for influenza infections. This drug acts by inhibiting the uncoating of the virus upon entry into the cell by binding to the M protein located on the viral coat. Therefore Choice C is the correct response.

Choice A describes Rifampin (TB and prophylaxis upon H. influenza or N. meningitidis exposure)
Choice B describes Oseltamivir and Zanamivir (Influenza)
Choice D describes Ribavirin (RSV)
Choice E describes Acyclovir and related drugs (Herpes) Recall that acyclovir is dependent on the viral thymidine kinase to become active while gancyclovir is not reliant upon this kinase. CMV lacks thymidine kinse so gancyclovir is Rx of choice for CMV
Choice F describes Zidovudine and related drugs (HIV)

plus it is NMDA partial antagonist and also works as an anti-cholenergic.
