hello dear

in case of restrictive/obstructive lung diseases whereever ther is hypoxemia of lung blood is shunted towards more healthier part of lung. ( imp point to remember here is the only organelle whoses vasculature undergo VASOCONSTRICTION under hypoxia is the LUNG. remainging all organelles under hypoxia undergoes VASODILATATION )

so lung always try 2 pump blood 2 its healthier part. when having lung pathologies all the blood is shunted to pulmonary vessels causing pulmonary cappilary hypertrpphy, leading to pulmonary hypertension, pumonary edema. this in turn causes right heart failure (backwarded heart failure).this is called COR PULMONALE.

Let me Explain you in another way.

Imagine a patient with pickwickian syndrome (Extremely obese).patient's lung do not expand enough during inspiration due to excesive fat --> hypoxia--->sensed by carotid bodies-->pulmonary capillaries vasoconstriction--->pulmonary hypertention-->pulmonary edema-->Right ventricular hypertrophy --> and eventually Cor pulmonale(or right sided heart failure due to pulmonary disorder).
Hope it helps
