A five year old immigrant child presented with hypertension and abdominal pain. On physical examination you note there are enlarged masses over the flank. Abdominal ultrasound reveals multiple cysts in both his kidneys. Which of the following is most likely true for this patient?

A. Long hands and hyper-extensible fingers
B. Narrowing of the proximal part of the aorta
C. Decreased field of vision
D. High pressure in the splenic vein
E. Hypertrophy of one side of the body

Default Answer is D

Good job. ARPKD is associated with congenital hepatic fibrosis and beyond neonatal years can present with Portal hypertension.

Yeah, I feel like such an idiot. I'm preparing for step 2 and studied just a week ago that ARPKD has a 50% incidence of hepatic involvement with bile duct proliferation and cirrhosis. Sometimes you know the sh+& but fail to see it.

It happens to me too. Framing what you read in a question form helps you remember things, for me it does. And an important thing is to keep on revising everyday.
