Question confused about usmle scoring

kindly tell me is this true that one's test result is based on how may students appear on the day of your exam all over the world n more importantly how well they perform? so is this sort of comparative analysis for a single day of examination?

other point is...
i read in this forum discussions that change in 3digit score corresponding to 99(or any 2 digit score) is due to different difficulty level for on individual exam day (or for each of the examine) so sometimes 228/99 or other times 228/97 (even i heard 227/98 n 230/93 among my friends)..
SO NOW MY Q is why is this so? they should have a standardized difficulty level for every single exam as well as standardized analysis of result? (i mean no1 gonna ask us "ohhhh u got 227/97 because you had a hard one or you got 229/99 bc u luckily got the easy one")....all they have is our 2/3 digit score and oon that basis we will b dont you think its sort of unfair game??

The two digit score system is going to end in July.
