Question cause of lacunar infarction?lipohyalinosis or carotid atherosclerosis

Lacunar infarction secondary to Myocardial infarction is due to small vessel lipohyalinosis according to uworld.
It can be due to carotid atherosclerosis but uworld says lipohyalinosis..
Any views regarding this?and how to approach such type of questions if 2 seem to be correct...:sorry:

I got this question wrong too because I picked carotid atherosclerosis but I read on a few different sources that Lacunar infarcts are much more commonly due to Lipohyalinosis secondary to Hypertension than thrombi from the carotid.
