MusculoSkeletal Causes of Proximal Muscle Weakness

Proximal muscle weakness with it's inability to comb the hair, inabilit to rise from sitting, ...etc is a common description in CK questions.
It would be a good idea to keep a high yield list of the most common causes of proximal muscle weakness.
Here's the list that I came with:
  • Inflammatory myopathies, such as dermatomyositis
  • Thyroid disorders (specially hypothyroidism)
  • Steroids, cushing's disease for example
  • Statins therapy
  • Cyclosporin
  • Hypokalemia
  • Myasthenia gravis
Please post your replies if there are condition that I have missed.

@ examzzz
Many of what you mentioned is "distal", Rasheed is talking about proximal muscle weakness only!

I don't think any of them except one causes proximal muscle weakness specifically.
They might cause proximal muscle weakness in part as a generalized disease process but so does death.
