anti double strand DNA (Highly specific for Systemic Lupus Erythematosus) is the answer if ask about disease activityor exacerbation. Serial assessment of anti-dsDNA antibody levels, is a sensitive and reasonably specific method for predicting disease exacerbations in SLE.

I second Miss Patho's explanation of the clinical use for anti-dsDNA. A couple of things to add:

Anti-dsDNA antibodies can be associated with renal involvement.

Anti-Smith antibodies are very specific (as much or more so than anti-dDNA antibodies), but only seen in 30-50% of SLE pts. Anti-Sm antibodies generally remain positive when titers of anti-DNA antibodies have fallen to normal or undetectable levels and clinical activity of SLE has waned. For this reason, they are particularly useful for diagnosis.
