Blood Hematology Peds Question

A 13 year old boy is brought to your office by his concerned mother. She states that her son has been sick for the past 3 days , she says he has a fever, fatigue and this morning was very disoriented. You exam the child and find a pale looking boy, with a few purpulish bruises on the skin and dry mucous membranes. Mother says he has not been able to eat much. His vital signs are HR: 90 BP: 110/60 RR 17 T 37.5 . His blood work shows RBC 3.7
Platelet count : 90,000 Peripheral smear shows hemolysis. Urianalysis is pending. Which of the following do you suspect is the diagnosis?

A. HUS
B. HSP
C. ITP
D. TTP
E. Bernard-Soulier syndrome

it's TTP (HUS+confusion/nervous system symptoms +fever)

D. TTP because neurological sign the diffrentiate it from HUS

Answer is D. TTP because of the CNS changes . the urine analysis would shou proteinuria and acute renal failure would be see n also
