Question A 3 week aold baby girl is admitted

A 3 week old baby girl is admitted for failure to thrive, diarrhea, and a sepsis like picture. She does well on intravenous fluids;when begun on routine infant formula with iron, her symptoms return. It is sunday and the laboratory is closed. You should begin feeds with which of the following?

A. Amino acid-based formula
B. Low phenylalanine formula
C. Low iron, routine infant formula
D. Soy based formula
E. Low isoleucine, low leucine, low valine infant formula

Default diagnosis of exclusion!!!!

definately not a nd b
i would have thought galactosemia bu that is not n option.so my best bet is soy based formula

The correct answer is D. Soy based formula . The patient appears to have galactosemia uridyl transferase deficiency is the cause and the condition results in features of jaundice, hepatosplenomegaly, vomiting, hypoglycemia, seizures, lethargy, irritability, poor feeding, failure to thrive, aminoaciduria, liver failure, mental retardation, and increased risk of E. Coli SEPSIS. Children with galactosemia are managed with a lactose free formula.

The low phenylalanine formulas are for PKU. the low iron formulas serve no purpose other than causing iron deficiency anemia; for patients with maple syrup urine disease and the amino acide based formulas are excellent for children with malabsorption syndromes.
But for Galactosemia it should be soy based formula
