If the pt and his family member declined the announcement of his disease, what the physician should do?

what do you mean by "declined the announcement of disease"?
If they don't want to hear about the diagnosis at all then the family's wish is not considered (because we don't reveal the diagnosis to the family. If the family does not want the patient be told of the diagnosis, then we must inquire why he doesn't want it to be told.

And the patient must be counselled and told that it's his body and only he can make decisions regarding his health and that it is important for him/her to know about the condition.
