Arrow Just out of the exam

Finally done .... Its really a tiring and exahsting exam , left around 5 qs blank due to lack of time, otherwise felt doing well or i hope so . Alot of energy and patience, u like start ignoring ur body feelings so u can continue, its very stressful and painful experience but finally over. Now I can just wait the result with alot of hopes.

Thanks all for ur nice wishes.

Preparation
I used FA in my preparation as main source, fastly read kaplan medicine topics with FA and did kaplan surgey (one day) and psychairty (3 days). Time pf preparation around 2 months home studying. Did uw offline (were scoring avg of 70%), nbme 4 offline and 2 blocks of kaplan simulated exam (67%,80%). I didnt see anything strange or like didnt see before in FA material in the exam. I also was going bk to read from the FA for step 1 so u can say I did that one too, I dont say its recommened but as it was still fresh in my mind,it was still recalled and wanted to refresh it.Revised FA in the last 10 days before the exam, I think this is the most important part of the preparation. Try to do at least 1 timed block the day before the exam, u will get in the exam atmosphere in faster way.

Before the exam
Exam is too long, get good rest the day before, dont study the afternoon before the exam and try to have something far from books and studying, this refreshing part without studying is very important in organizing ur thoughts and informations and preparing urself for the exam day and will help u to sleep bcz thats the most challenging part the night before the exam. Dont wake up too early on the morning of the exam , you need ur energy to be working and preserved as late as 5-6 pm. Dont be late and leave time (around 30 mins) safe time for any emergency. Dont forget ur ID and permit, sometimes ID causes problems they prefere passports and driving licences more than local country IDs. Wear the most comfortable clothes and shoes, being dressing uncomfortably is the last thing u need. Having some muscle relaxant and pain killer pills with you isnt a bad idea, I felt sort of leg swelling and very bad neck pain at the end of the exam.

Exam:
The first block is challenging as still ur not knowing how fast u should be to end on time. Especially when u face this type of long stuff question in the beginning and tempting you yo to do and lose valuable time on it. Dont give any q more than 2 to 2.5 min in any case, but dont just answer anything in hope u will be able to come bk to it, u hard will be able to, so just give it more few secs, see whats more close answer and go on. After the first one u will know how fast and the speed u have to go through to finish the block on time. Good method u can appreach, that u put a target that u should be on question 22-23 at 30 mins left , it worked with me.
IMPORTANT note: always scroll down the left side bar in the beginning of the exam, so u can see the number of questions in the block and how ur proceeding, this esp imp for those doing there exams after Augest 3 as the blocks wont all have the same number of questions

Questions:
There will be few questions that doesnt need more than 20 to 30 secs and save u time though dont expect much of them, much less than step 1. Most (More than half, maybe tow thirds in some blocks) the questions are uw longest questions type and longer, and much has those list of investgations listed, best approach is to go through fastly see whats abormal and save it in ur mind. YOU should know normal values ranges ( Elecrolytes, TFT, LfT and RFT (everywhere uc these ones), else u will face a problem looking back and froth at inv page and no time for this. I was doing a fast reading for the question first time, in case -as sometimes u know the end will be totaly different thing- I can solve then it is,if not i go and read it again ( Esp in whats next step qs or best managemnt needed), bcz I found it hard to do it from first time as its long and u wont approach the key points unless u read the whole questions and get idea about what the question needs. Most of the blocks have 1-2 sequential items, actually most were of that type were the list of choices are the same for more than 2 case csenarios, i liked it usually short and easy but know ur answer as usually there are more than 10 listed u cant go and read and start searching ,guess ur answer and pick it up, very very few (less than 3) were of the type that second question depend on the first and the first will lock when u proceed. First will be long, second short, the good thing -maybe bad- that u will know that u get the first question right or wrong.

Exam Components:
Medicine as known occupy the scene in the exam. Alot of rashes pics, few xrays, 1 or 2 MRIs and CTs, had 2 or 3 audios of heart murmurs werent hard. Obs fetal heart monitoring thing diagrams, some gyn. 9 PAP smear, STDs, cancers), pediatrics second to medicine , 3 or 4 develpoment qs not easy ones, psychiatry with its very long story qs, epidemiology less than 5. There was surgical questions, uw cover it. And for sure some 5-10 peripheral nerves questions I was able to asnwer from step 1. Orthopedics (Esp pediatric age hip diseases), toxicology ( Substance abuse scenarios and antidotes qs), ophthalmic diseases ( werent few) and ENT ( few).

Break Time:
Actually managing the break depends on the person and his situation after that particular block in the exam. But as a role I dont advice at all to do 2 blocks in sequential it burns ur energy for the whole exam, the last questions of the second block will be very hard on u, I tried it without break and seat break and tried 3 mins outbreak the later much better and make u be refreshed for the next block in much better situation and survive the whole exam. Take anything during these small breaks water, chocolates and juices ( excellent fast sugars) I felt it immediatly affecting and refreshing and providing energy source for my mind.and body Have at least one long break (15 mins) have some small sandwiches with the sugary things. When I saw I dont have much break time, I could save some mins from 2 blocks and added it to my break time and gd I did so, I could use it between my last blocks much needed that time.

I like usually to write my experince after Iget my result, but as its my last step among the three and upon request of the many of you who are having there exams soon; I write it now.Maybe much details but any the little tip or detail is important for this exam. Final advice try ur best to be relaxed and fast, stress and delays (esp in the beg. of the block) is the most things that can affect performance during the exam.
Best of Luck

Default congratulations drmuh and arunjose!

Finally step-2ck is over for u guys(i have mine on 23 august)!
Good luck for your score!!!

Can you share your preparation details (uw%, any other qbank or materials used, etc..) and what areas the exam focused more on (eg. more Diagnostics questions, etc..). How was the length of the questions? how many long questions, medium questions, etc.. and how would you rate the difficulty compared to Uworld (for example, on a scale of 10).

One final request guys please post your uworld score from the "cumulative performance" section of the qbank. Please post this as i am unable to assess my performance and want to compare with you guys performance sheets. You can post it here in the thread.

You rocked the exam guys! party time!

Btw in the survey at the end of the exam, there is a q if u prefere if this exam was in 2 days. i Encourage ppl to start asking for this. This exam doesnt measure knowledge as much as it measure maybe tolerability for stress and pressure as its a military mission not exam to see how much knowledge the doctor has. I know thats a part of any exam, but not the target of any exam which I feel the case here. I think highly knowledagble doctors and specalists wont find it easy even to sit for this exam

hey drmuh!
what about the psych questions? people say that there are many "weird" questions in the exam. is it true? I am really bad at psych. Are there many questions? is kaplan and uworld enough for them?

To be honest I was good at psyc. was having in the 80s in uw. But the exam psyc. questions are of the long type, more like a story thing. I dont think its matter of studying more books ( I did FA and kaplan, if u like u can add the behaviroal HY psych chapters to ur studying plan) as much as doing many questions from different sources on this topic (uw isnt too close to the exam ques type in this topic). I advice you when u face this type of questions to summarize the key points and pick the most appropiate psych. disorder that match and cover all of them (for example if u have both depression and psychosis think of shizoaffective disorder and etc). The most difficult ones maybe the pyschosis ones , more than the stress and somatiz. disorders. Yes every like 10 to 15 q u will have psych. question like this, its looks long and weird but if u read it again u will the find the key point/s that help u to solve the question. Didnt get much ethics questions, actually very few, much less than step 1

I read kaplan, but for trauma thing and surgey I advice you to do the emergency chapter in FA its really good covering it. You can do it in half a day and read the review and look at the pics at the end of the book. If you u dont have it u can download it. And yes uw covers it in gd way.

IF u did FA earlier then review it in the last 2 weeks. If u did kaplan it will be hard to go through again and its not needed but u can review kaplan medicine only. Last day before the exam do one timed block in serious way as its an exam, it will help u alot in the exam day, rest of the day get enough rest and pleasure away from ur books.

Good Luck

hey, on this website, discussion and posting on NBME question is not allowed. I recommend you to go to http://usmleforum.com/ where all questions are discussed openly and you will find your nbme 4 questions there.
good luck!

I took my CK today and was so relieved to see that I didn't get any of the new questions. I had rescheduled from the 15th to today just three weeks ago.
As an IMG with English as my second language I was a little worried about the timing issue despite the fact that I am fast reader. I did manage to answer all questions and had at least 5 (up to 15) minutes time at the end to review questions that needed further thinking.

overall i agree that you need to take the last couple of days before the exam off to collect some energy, because it is very tiring.

I felt like I had many psych questions and quite a few pediatric psychiatry questions but immunology/infectious disease dominated the exam. Good luck to everyone!
