Angry CVS Question.... ATP 3 guidelines

there is a much buzzling about that

guidelines propose u ll follow step by step as they recommend,......
-according 2 that i should calculate framingham score- 2 assess if a patient without CAD or its equivelant ll follow which category ...
i.e
if in a pt without CAD equivelant 10 year risk of CAD >20%.. (giving framingham total score >17) .. this pt should be ttted as v.high risk ie at LDL >100 ...ttt ll be LIfe style,
at LDL >130 ...tt ll be statin ttt.............

also for moderately high risk whose 10 year risk 10-20% ttt will be different from those with moderately risk whose 10 year risk < 10%

even UW deal with a case by the same manner considering framingham score while assessing a pt next step in ttt...

even the supposed final dialogue of guidelines tt differ from that in FA in 1st category ttt inthat
UW ttt pts with CAD by statin at LDL > 100
while
FA start ttt in those at LDL > 130.........??

i think a q in this matter ll never not be in an exam....
though highly conflicting...

then my Q is how we can assess framingham score in the exam.........????!!!

Dr fisher said in MTB about this topic. He said every hospital has different datas when it comes to LDL level . But national guideline for statin treatment and LDL level is >100 in pt with CAD/ CAD equivalent.

Hi i could not find that score in my FA..I think because of old edition.

Can u please tell page no in FA and edition year......

I have not seen that in kaplan too?

noooooooooo it is latest guidelines ...check this PDF carefully .., the score i mean is al last page ... but follow step by step 2 get what i mean..

man ..this score is NOT in FA .... but UW Q used it as a tool 2 answer a case which is NON CAD or its equivelant........ but by framingham score it is considered to have 10 yrs risk > 20% ...so this guy cannot be categorized -as FA consider any non CAD equivelant- moderate risk
butas high risk ... at which lipid lowering drug started at LDL 130

FA table ... simplifies the issue so much by ommiting the need 4 the score
but i was faced by a Q which has no CAD equivelant but LDL 140
UW answer presumed calculation acc 2 framingham considering points >17 so considered high risk ...giving him statin at that LDL
while acc. 2 FA this guy should intensify dietry changes ONLY
