Heart Echo or Blood Culture for Subacute bacterial endocarditis

Subacute bacterial endocarditis

what is the initial test (echo or blood culture ) i am confused
whats the most accurate test ???

Initial test should be blood culture since it helps to know and isolate specific organisms and can start specific treatment.

Echo is mainly to see structural damage caused to heart i,e to see any regurgitations, stenosis etc. I think which we can do after starting with blood culture since specific tx which we started depending on blood culture helps decreasing diseases or modify disease course. As well in seriously ill patient tx should started first so blood culture should be initial test.


If in case blood culture is negative then it indicates patient is on antibiotics and we proceed with echogram to locate vegetations, abscess etc

I am also confused about most accurate test: blood culture has 99% sensitivity so i think it is most accurate test.

blood culture should be the initial test while echo would be the confirmation test hence echo being more accurate

This might be a stupid question..... How will blood culture rule out any other source of infection??
