yes, endoscopy is not sensitive to Dx perforations. If you have Sx of mediastinits or an effusion or subcutaneous emphysema or sharp tearing pain sub-sternally, then only it would be suitable to do gastrograffin swallow.
For all other cases, after initial stabilization of the pt. w/I.V fluids, do endoscopy to evaluate for mucosal damage like strictures, etc.. and carry out possible therapy.
