Question Most Accurate Test for Wegner's Granulomatosis!

what is dth most accurate diagnostic test for Wegner's granulomatosis?
1) biopsy of nasal septum (Kaplan CK Notes)
2) biopsy of lung (MTB-CK)

I'd go for nasal septum! because the lung is extensive....so it'd be hard to find the correct place to biopsy and because its so empty because of the air, we'll have loads of false positives.

actually this is clearly stated in MTB-CK....wegener GM most accurate DG test is lung biopsy....Renal and nasal septum are inferior to lung...So it is lung biopsy if those others are included in the choice...

actually there was a question in uworld dat said nasal septum and kaplan also prefers it so i was confused.MTb puts lung biopsy superior to renal and sinus biopsy..i guess sinus biopsy is not d same as nasal septum biopsy? wat do u say?

honestly it depends on the wording of the question...I would go with lung as the most accurate biopsy...

if anyone has anything else to add regarding inital Dx..please do so..

The answer depends on the case presentation, wegeners MAY involve both the UResp system as well as the Lower Resp system , and the biopsy is equally effective at either position IF THE FINDINGS of wegeners are present at those locations.

The lung is superior in a vague question because it is much more commonly involved than the nasal septum. however most of the cases given in USMLE would have commonest presentation which includes nasal bleeds and id definitely biopsy the nasal mucosa wudnt anyone? lung biopsies are mostly done during autopsy in WG

Biopsy of affected tissue. The only sure way to confirm a diagnosis of Wegener's granulomatosis is to remove a small piece of tissue from an affected organ (biopsy) and examine it under a microscope. remove tissue from your nasal passages, airways or lungs to confirm or rule out the presence of both vasculitis and granulomas. Other areas for biopsy may include your skin or kidneys. Some biopsies can be performed in an outpatient setting using a numbing medication (local anesthetic). Others, such as an open-lung biopsy, may require hospitalization., so its depend upon mcqs if test are equivocal on nasal biposy then we have to go for lung biopsy, but on mcq they will not ask in complicated way so get it easy........
