Question Best Initial Test and Most Accurate Test!

So I made a list of what I could not find, or had some doubt in..for the following..
please help in answering whatever you can. Thx

Best Initial Test (BIT) for:
Aplastic Anemia?
Essential Thrombocytosis?
Paroxysmal Noctunal Hburia?
Herediatrary Spherocytosis?
Gout ?
Pseudogout?
psoriatic arthritis?
Gonoccocal arthritis?
hairy cell lekemia?
MGUS ?


Best Confirmatory/ Most Accurate Test (MAT) for:
Essential Thrombocytosis?
Cryoglobinemia ?
Rheumatoid arthritis ?
Myelodysplastic syndrome ?
Myelodysplastic syndrome?
CLL?
Waldenstrom Macroglobinemia?

I was thinking that for AA:
BIT= peripheral smear=> pancytopenia
MAT= BM Biopsy.

Mostly blood diseases' initial test is CBC and periphral smear (first four )
arthritis - you do aspiration of joint except septic first emipric rx then culture from multiple site.
hairy cell leukemia- periphreal smear.
MGUS - main thing is to exclude myeloma - so do BM biopsy

So I'm just gonna drop some guesses here and plz let me know if I'm wrong. I welcome any corrections:.
  • Essential thrombosytosis, BIT= platelets count , smear?; not sure about MAT??
  • spherocytosis, BIT= smear
  • CLL, MAT= BM Biopsy
  • Waldenstroms, MAT= IgM spike SPEP? i dont know of anything else that could cause this.
  • Myelodysplastic syndrome, MAT= BM Biopsy

Essential Thrombocytosis?...................CBC-->PLATELET >500000
Paroxysmal Noctunal Hburia?................flow cytometry CD 55,CD59 , decrease DAF

Herediatrary Spherocytosis?.................i think osmotic fragility ,(CBC has no clue except spherocytes which puts a DD)

Gout ?..............................................J. aspirate(BIT & MAT)..AS I THINK
Pseudogout?......................................" " " " " " " " " " "
psoriatic arthritis?..............................?! i think by evident C/P of sero-ve arthropathy , coexist psoriasis , ccc radiological features

Gonoccocal arthritis?..........................uretheral culture bef4 ABs Rx
hairy cell lekemia?...........................CBC..?, smear...hairy cells suggestive
MGUS ?............................................eleva ted monoclonal Ig on SPEP


Best Confirmatory/ Most Accurate Test (MAT) for:
Essential Thrombocytosis? ...................BM biobsy...?
Cryoglobinemia ? .................................BIT=decrease complement ,,
..MAT = cryoglobulins

Rheumatoid arthritis ?........................clinical Dx 4 out of 7 ccc ,No single test 2 Dx ... but AntiCCP is specific but not found in All cases

Myelodysplastic syndrome ?..................
...........i find this .... The diagnosis is suspected on a bone marrow core biopsy and confirmed by routine cytogenetic study. Bone marrow biopsy shows uniformly small and monolobated megakaryocytes (arrows)

CLL?......................................BIT=CBC, smear is ccc by smudge cells, cytogenetics=CD 19 ,,,, MAT ...?

Pseudogout? Xray to show chondrocalcinosis
psoriatic arthritis? Im not sure but X Ray to show pencil in cup ???
Gonoccocal arthritis? Joint Aspirate
hairy cell lekemia? TRAP staining on CBC
MGUS ? no initial test can determine this diagnosis , this is a diagnosis of exclusion.


Best Confirmatory/ Most Accurate Test (MAT) for:
Essential Thrombocytosis? Marrow aspiration
Cryoglobinemia ?
Rheumatoid arthritis ? anti CCP
Myelodysplastic syndrome ? bone marrow aspiration
Myelodysplastic syndrome?
CLL?
Waldenstrom Macroglobinemia? Immunoglobulin Spike
