Default Pulmonary function Q

50 years old male presented with dyspnea & shortness of breath, is intubated in the intensive care unit, PaO2 is 70, BP 90/60, Hb is 8.0, what is the most import step to take to improve O2 delivery to vital organ?
  • IV isotonic saline to raise the BP.
  • O2 delivery through the ventilator.
  • Blood transfusion to raise Hb above 10.0
  • Dobutamine to improve cardiac out put.
  • EKG & cardiac enzymes.

This guy has a low BP and low Hb.....I think he is/was bleeding and right now he is close to get into a hypovolemic shock; in my opinion, the first thing to do is to give him iv isotonic solution, before administrating a blood transfusion or oxygen.....

The scenario is not giving us too many details.....did the question gave you more details or what you post is all you got???

in this case, the best step will be ' blood transfusion to raise Hb ' , however if question will be asked first step to do ten it will be ' iv isotonic slotuion to raise the BP '.

Why do u guys want to give saline to a pt with Hb 8 n b,p 90 systolic, when u have blood in ur hand...??

i think blood transfusion is the best answer...

n here they just want to tempt us in giving oxygen wrongly..!! because in a pt with decreased hb n cardiac outpu, even 100% o2 wont increase the po2...

i hope im right??

Just carefully read the question and then answer !

best answer is BLOOD TRANSFUSION !

If the q asked best initial step in management the the answer would be IV SALINE !

The correct answer is blood transfusion.
Raising the Hb level is the best way to improve O2 delivery to the tissues.
