GIT H. Pylori Treatment Follow Up!

A 28-year-old woman presents with a 8-month history of an intermittent burning sensation in the epigastrium that is sometimes related to eating. No weight loss and denies dysphagia. Laboratory tests are normal. The patient has a positive H. pylori antibody blood test. She is compliant with the medication. Her symptoms initially respond, 4 month latter she started to experience the symptoms again. Which of the following is most likely correct?

1.
She is at high risk for reinfection with H. pylori.
2.
Dyspepsia usually improves with H. pylori eradication.
3.
A positive serum IgG indicates that eradication of H. pylori was unsuccessful
4. The urease breath test is an ideal test to document failure of eradication.

Response to therapy is checked by Urea breath test.

You are correct, it is number 4.
The urease breath test is an ideal test to document failure of eradication
Either the stool antigen or urease breath test is ideal to document treatment failure.

can someone please clarify when is it best to use urea breath test and when is it best to use stool ag....??

As far as i remember, both r equally sensitive with same advantages, n can replace one another...
there main comparison is with serology which is quick, easy but cannot differentiate old n previous disease...!! so mainly preferred for dx but cannot be used to check failure or relapse...

n the best, most sensitive n most invasive is endoscopic biopsy...which is not only diagnostic, can differentiate old n new, but also tells abut malignancy...!!
