Lungs Severe asthma management

20 YO lady with history of severe asthma, steroid dependent, 5 times a day albuterol inhaler use, admitted many times to ER over the last 6/12, O/E wide spread wheezes. What is the best management for this patient?

  1. IV steroid and keep her on higher doses of oral steroid.
  2. Montelukast + steroid.
  3. shift her to salmeterol & add Montelukast + steroid.
  4. high dose oral steroid + albuterol inhaler.
  5. Refer her to asthma specialist for further evaluation.

Answer 3.
Give l Leukotriene modifiers (Montelukast ) to reduce the need for the steroid & shift him to long acting beta agonist (salmeterol) + steroid.
