USMLEWorld USMLE World Categories of Internal Medicine Questions

I have a simple question.

Are the questions in USMLE world ONLINE Qbank, topic wise in Medicine or are they altogether random? like as hepatology, neurology, cardiology or just labeled under Internal Medicine?

If you pick Internal Medicine, then that will include subspecialties such neuro, cardio, nephro, endocrine, ...etc

u can choose any single subspeciality in IM, in a single test...!! or u can mix them all...!!
