Virus Post exposure Prophylaxis for Hep and HIV

I'm looking for summary for management of post exposure for hepatitis A, B, C & D as well as HIV, would you please help?

Hep A: administer HepA Ig, obviously not necessary if previously vaccinated.
Hep B: administer HepB Ig + start Hep B vaccination schedule, if not vaccinated.
Hep C: I'm not aware that any postexposure prophylaxis exists, correct me if I'm wrong.
HIV: administer triple therapy (same regimen than initial HAART recommendation) for 4 weeks.

I wana add for hep-A :- if vaccine is given in <30 days then give Ig.

If vaccine is given 1yr back and patient is exposed to hep a or b ..what should we do only immunoglobins that is passive immunization or since its 1 or 2 yrs back so we should give both vaccination and immunoglobins??

And if same patient with 5 or 8 yrs after vaccination presents after exposure??

How we proceed in each case?
