Default systolic ejection murmur with wide fixed splitting of S2

4 years old boy, at dental clinic, on routine chest examination, systolic ejection murmur, along the left mid to upper sternal border, load S1 & wide fixed splitting of S2. Otherwise the child is normal. Which of the following statement is correct about this case?
  • The child has VSD & needs antibiotic prophylaxis.
  • The child has VSD & does not need antibiotic prophylaxis.
  • The child has ASD & does not need antibiotic prophylaxis.
  • The child has ASD & needs antibiotic prophylaxis.
  • Endocardial Cushion defect & needs antibiotic prophylaxis.

i would go with asd and does not need a prophylacticantibiotic before the dental workup

We prophylax when
1) significant cardiac defect specifically
prosthetic valve
transplant recipient
previous endocarditis hx
or unreparired cyonotic heart disease

This sounds liike a simple ASD which doent need prophylaxis

The correct answer: The child has ASD & does not need antibiotic prophylaxis.
It is low-flow lesion, no need for prophylaxis.
