Default Rheumatology

A 65 years old man with psoriasis has been troubled with long standing destructive arthritis involving the hands, wrists, shoulders, knees and ankles . Hand films demonstrates pencil in cup deformities . He has been treated with naproxen 500 mg bid, sulfasalazine 1 g bid, prednisone 5 mg qd, and MTX 17.5 mg once a week with ou substantive improvement which of the following is most likely to provide long term benefit ?

a) cyclophosphomide.
b) addition of folic acid supplmentation.
c) oral cyclosporine.
d) TNF alpha inhibitor.
e) higher dose prednisone in the range of 20 mg per day

Any DMARD would benefit long term morbidity so my choice would be biologicals... TNF inhibitor... Cyclophosphamide is a distractor here... Whats the answer?

The correct answer is d) TNF alpha inhibitor.
His man has failed conventional DMARD therapy. TNF alpha blockers have shown dramatic benefit regarding both arthritis and skin disease.
