Question treatment difference between E Coli O157:H7 and Shigella

Hi all:

To treat O157H7 E.Coli Hemolytic Uremic Syndrome, antibiotics are not indicated due to the possibility of releasing more toxin from bacteria, just like
Salmonella enteritidis. However, can someone please explan why the treatment for Shigella is to give third generation cephlosporin?

Thank you!

diarrhea management depends if its invasive or not.
both Hemorrhagic and shigella will give invasive and so you will give ciprofloxacin. for diarrhea, i guess you wont come to know the specific organism ..you will just order the stool for cells,ova-parasite and WBC. thats it. i doubt if you will send it for culture. and by that time[ 3-7 days for culture report], patient will be okay.

in HUS, you avoid antibiotics.
