Default Please help with a few ethics questions on step2CK

1) The patient does not speak English and you have an interpreter coming in. After greeting the interpreter what should you do. All the other choices are obviously wrong such as practicing with interpreter etc. I came down to between:
A. Remind the interpreter to speak directly to the patient.
B. Briefly discuss the expectations of the visit with interpreter.

2) A guy came in being really sick and require mechanical ventilation. Wife said he doesn't want it but has no document while daughter wants everything done. There was no choice to talk to them and get them on the same page. C is the only stalling option as I remember.
A. Do what the wife wants
B. Do what the daughter wants
C. Put in endotracheal tube then consult ethics committee.

3) A guy with gonorrhea having unprotected sex with his partner and asked for confidentiality. There is no option to ask him to tell his partner.
A. Agree with him
B. Tell him you are required to tell his partner.

4) A 16 y/o girl who had 4 previous sex partners and currently with inconsistent condom use. Whats the best prevention for her agaisnt HPV.
A. Abstinence
B. HPV vaccine.
C. Condoms? Not sure if this answer was there now.

1) I want to know how was the question phrased. Ethics questions can be quite tricky. But given how you phrased it, then it should be A.

2) A. Do what the wife wants, because what she wants, is what her husband wants in the first place. A written documentation is not required. It is considered a oral advance directives

3) Theoretically, convince him to tell his partner. Next invite them over to your office. Are you really sure that these two are the only possible answer? Because if so... then the lesser evil would've been tell the partner. Since infectious diseases are still considered threat. And we all swore to do no harm.

4) If the question asks, what is the best... then probably the answer is abstinence. Because if you don't have sex, then you obviously won't get HPV. But if the question is what would you advice (meaning you consider practicality) then you answer HPV vaccine. But it will not protect you really with HPV. It is only protective of common HPV serotypes. Sadly, there are more HPV serotypes that are circulating. So getting vaccine is not a fool-proof method.

Ethics is my waterloo. So I am as clueless as you are...
